Good Faith Estimate (No Surprises Act)

Under the No Surprises Act (effective January 1, 2022), health care providers are required to provide a Good Faith Estimate (GFE) to individuals who are uninsured or who are not using insurance for services.

If you are not using insurance or choose to self-pay, you have the right to receive a Good Faith Estimate explaining the expected cost of your care.

What You Can Expect

A Good Faith Estimate will include an overview of the anticipated costs for therapy services based on your individual needs and treatment plan. Because therapy is individualized, the total length of treatment and number of sessions may vary depending on your goals, progress, and clinical needs.

Your Rights

You have the right to:

  • Request a Good Faith Estimate before scheduling services or at any time during treatment
  • Receive a written estimate of expected charges for therapy services
  • Dispute a bill if it is $400 or more above your Good Faith Estimate (per federal guidelines)

Important Information

  • The Good Faith Estimate is not a contract or guarantee of total cost
  • Your actual cost may differ depending on the frequency and duration of services
  • This estimate does not include unforeseen or emergency services that may arise during treatment
  • Insurance billing (if applicable) may differ based on your plan coverage, deductibles, and co-pays

Requesting a Good Faith Estimate

If you would like a Good Faith Estimate or have questions about service fees, please contact:
